This notice describes how information submitted through the 4th P OS individual application and organization inquiry is handled.

Information we collect

The forms collect the contact information, organization information, circumstances, goals, access barriers, readiness, and partnership details that you choose to provide. Please do not submit government identification numbers, financial account details, medical records, or passwords.

Why we collect it

Information is used to review access applications, evaluate referrals or partnerships, contact the person who submitted the form, operate the program, prevent abuse, and meet applicable legal obligations.

Who receives it

Authorized people working for or on behalf of the 4th P may review the information. Service providers used to receive, store, secure, or manage submissions may process it under their applicable agreements. Application information is not intended for sale or advertising profiles.

Retention and choices

Submissions should be retained only as long as reasonably necessary for application review, program administration, legal obligations, security, and dispute prevention. To ask about, correct, or request deletion of a submission, contact hello@the4thp.org. Some records may need to be retained when required by law or legitimate organizational obligations.

Security and updates

The site is designed to transmit submissions through an encrypted connection to a nonprofit-controlled destination. No online service can promise absolute security. This notice will be updated when the final submission processor and retention schedule are approved.
